I don't have a scanner so I was so happy to find this image. I wouldn't say it looks pretty close to the original - I have the CD right in front of me and I'd say it's IDENTICAL! Awesome quality, thank you so much!
I have the European 2012 edition which was part of the limited edition boxed set, and most of the track names are all in Romaji. Below are the tracks exactly as printed on the CD case:
